Dr. Elariny’s concierge access
The concierge service is associated with Dr. Elariny and supports continuity and access during preparation and recovery. Enrollment for the first year is required just before surgery is scheduled. Renewal after that first year is optional; annual renewal is not required.
Ask the office to explain the current subscription price, included access and services, and how it relates to surgical and follow-up charges. Ongoing medical follow-up remains important regardless of whether you renew the subscription.
Insurance and separate charges
Out-of-network benefits vary by plan. Ask your insurer about bariatric coverage, out-of-network benefits, authorization requirements, deductibles and reimbursement for the proposed procedure. An authorization or benefit quote does not guarantee payment.
Clarify the surgeon’s fees and any separate facility, anesthesia, laboratory, imaging or other clinician charges. The network status of one participant does not establish the status of the others.
Questions to resolve before scheduling
- What services does the written estimate include, and what is billed separately?
- What payments are due, and when?
- What are the cancellation, rescheduling and refund terms?
- Who submits insurance claims, and what documents will I receive?
- What access and follow-up are included during the first year and afterward?
